Rochdale Borough Council's STARS adult care service has been rated good by the Care Quality Commission following its latest inspection.
The service supports more than 1,200 vulnerable residents each year after they leave hospital, helping them remain independent at home.
The latest assessment found the STARS service to be a well led service delivering effective, caring and responsive support for people living in Heywood, Middleton, the Pennines and Rochdale.
The Care Quality Commission said the service provides a high standard of care and ensures people receive support that meets their individual needs.
Inspectors praised staff for treating people with kindness, empathy and compassion while helping residents live healthier lives and maintain their independence, choice and control.
The report also highlighted the service's focus on placing people at the centre of decisions about their care, effective partnership working and strong communication with families, external services and other care providers.
The assessment follows the council's wider adult care service also receiving a good rating from the Care Quality Commission.

I know from talking with my constituents how much they value STARS for the compassionate and high quality care and support they provide for vulnerable residents.
"Adult care is about delivering high class services that help people remain independent in a place of their choice and there is no better example of this than STARS. I would like to thank members of the STARS team who are doing such an amazing job. The CQC assessment is deserved recognition of their outstanding work. I am so proud of this service for supporting thousands of residents, helping to reduce health inequalities and free up valuable hospital beds."
The council said the STARS service has been supporting residents across the borough since 2008.
